  • New Patient Packet - New patient paperwork packet (Save file in PDF format and email completed forms to info@umpquaortho.com)
  • Medical Records Send Request – Complete this form to obtain a copy of your Umpqua Orthopedics medical records. As per OAR 847-012-000, medical records requests will be completed within 30 days from the date the signed request is received in our office. You may be charged a reasonable fee for reproducing your medical records, and payment is due in advance.
  • Medical Records Obtain Request – to obtain records from another physician / facility (Save file in PDF format and email completed forms to info@umpquaortho.com)

In order to enter your information into the patients form above, you would need to download the form to your Phone or PC. Once Downloaded, you will need to use a PDF editing software such as the Adobe app to make changes to the Forms.
